Creating a Survey

New Quizzes supports survey-type quizzes, allowing you to collect anonymous or non-anonymous feedback, with graded or ungraded options. Questions can be set as required or optional. You can view survey data in the Reports tab.
Note: Surveys created in Classic Quizzes can be migrated into New Quizzes. However, in order to edit the Quiz Type and Anonymous Submission settings, consider re-creating the Survey. Migrated surveys do not allow you change the type or anonymous submission settings.
Step by Step Instructions:
Select Quizzes from the Course Navigation menu on the left side of your MyClasses Course.
Course Navigation menuSelect +Quiz/Survey in the top right of the screen.
Begin creating your assessment by customizing the following:
Enter the name of our survey as the Assignment Name
Select the Quiz Type as either Graded Survey or Ungraded Survey.
If a graded survey, establish the number of points in which the students will earn when the complete the survey.
Determine whether you want the survey responses be anonymous to your or not. Note: Once anonymous submission is turned on for a survey, it cannot be turned off.
Example settings of a graded survey in New QuizzesFor Assign Access, identify who can take the survey under "Assign to", typically everyone.
Specify the due date
Specify the dates in which the survey will open to students (Available from) and close to students (Until).
Select Build to create or edit the survey.
Save and Build buttonIf desired, click the pencil on the right to edit the instructions for the survey. Be sure to select "Done" once you're finished editing.

Multiple Choice
Multiple Answer
True/False
Fill in the Blank
Essay
File Upload
Once you have added all of your questions, you can adjust your Settings or click the Return button in the top right to return to your MyClasses course area. Once all questions and settings are established, remember to Publish your survey.
